SLT Member Survey for Biennial Review Process

The Biennial Review survey is important to provide information to each district concerning how well SLTs are functioning according to State mandates.Each district then has to send its report to the State.It is important as a School Leadership Team member to be honest in your responses so that the problems concerning our SLTs can be documented.

Biennial Review Survey

Dear School Leadership Team Member,

In accordance with Commissioner’s Regulation 100.11, an evaluation of SLTs will be performed every two years as part of a Biennial Review process. This survey has been designed for you by our office in consultation with District Leadership Team (DLT) members to provide candid information about the overall effectiveness of School Leadership Teams (SLTs) and your experience as a member of a team. Your full participation in team activities ensures that stakeholder voices are heard and considered in the important work of educational planning and shared decision-making at the local school level.

The questions in this survey correspond to DLT priorities as outlined in Commissioner’s Regulation 100.11. With your responses, DLTs will collect and process information from each school, allowing a fair assessment of district-wide implementation of the regulation based on actual team member feedback. The next Biennial Review must be completed by February 2010.

Please begin completing your survey by clicking the link to your district below. All surveys must be submitted by January 15, 2010 to ensure that DLTs have sufficient time to review the data and complete their review.
